To ensure all the information in the report is an accurate reflection of the facility, a couple of actions are required:
- The Verifier discusses findings with the facility and resolves any issues before submitting the verified data on the Accredited Host platform, preferably during the closing meeting of an on-site verification.
- Once the Verifier completes the verification online, the facility is notified via the Accredited Host platform and can access the verified assessment report online or offline. The facility then has 14 calendar days to review the report, raise any question level issues with the Verifier/ Verifier Body, formally dispute the report, or approve it.
Note: any changes the Verifier makes to the report after completion/ during the 14-calendar day facility review phase must be agreed upon by the facility. The facility must be informed about the changes and when they took place, so the facility can go back to the review (with the implemented changes) and accept the verification before the 14-calendar review period is over.
Note: after the Verifier submits the verified data, SLCP will conduct an automated quality check. Verifiers will be contacted by SLCP or the VOO if the quality check indicates that changes need to be made. This would happen in the first couple of days after submitting the verified data, before the verified data is sent to the facility.
