The Data Collection Tool is the ‘Questionnaire’ used to gather data on working conditions in a facility and used by the Verifier to verify this data. It must be downloaded by the facility from the AH platform to start their self/joint-assessment. The Data Collection Tool on the Helpdesk is for information purposes only.
