The Data Collection Tool is the ‘Questionnaire’ used to gather data on working conditions in a facility and used by the Verifier to verify this data. It must be downloaded by the facility from the Accredited Host (AH) platform to start their self/joint-assessment. The Data Collection Tool on the Helpdesk is for information purposes only.
The CAF v1.7.1, including the Offline Data Collection Tool, s available in six languages: English, Chinese, Turkish, Vietnamese, Latin American Spanish, and Italian. English is the primary version and the source of truth: all self-assessment answers must be submitted in English. The translated versions are designed to help you understand the questions and guide you as you answer them. If anything in a translation seems unclear or inconsistent with the English version, defer to the English text.
