No. You can not change or add information in your verified assessment report. After the report is finalized no more changes can be made.
If errors are discovered by a facility, Verifier Body (VB), or the VOO through the Quality Assurance (QA) or other processes, an addendum can be requested. Addendum must be requested by the facility through this form. After the facility sends a request, the form is sent to the VB for completion and then routed to the VOO for final evaluation and approval. Addendums are only intended to update incorrect facility and Verifier selections that were made during the last Verification. Remediation or corrective actions taken by the facility after the verification will not qualify for an addendum. The addendum is included by the VOO in the Gateway Report section "Assessment Information". Find all rules around addendum requests in the QA Manual.
Note: when the conditions in your facility have changed significantly since your last SLCP assessment, please start a new assessment to come to a set of verified data that accurately reflects the conditions in your facility. There is no need to wait until a year has passed.
